02.Housing Support Program
This service offers eligible members rent and utility assistance for up to six months, aimed at stabilizing housing situations and promoting safety. It includes assessment, application assistance, and follow-up support to ensure households receive timely aid. The program also provides safety and health home modifications to improve living conditions.
03.Intake & Enrollment Support
This service guides individuals through the process of enrollment in Oregon Health Plan (OHP) and related programs. It provides assistance with documentation, eligibility verification, and understanding program benefits, ensuring a smooth intake experience. The service also helps connect clients to referral partners like clinics, shelters, and county programs for comprehensive support.
