What is HRSN?
HRSN stands for Housing and Outreach Support Network. It is a state-funded program designed to help people experiencing homelessness in Oregon transition into stable housing through a combination of rental assistance and outreach services.
Who qualifies?
To qualify, you must have Oregon Health Plan (OHP) coverage and be on the OHP open card (fee-for-service) program. You are also eligible if you are in a qualifying life transition, such as housing instability or leaving incarceration, with a related health or social risk factor.
What does open card mean?
The OHP open card program is a fee-for-service benefit. It allows you to receive medical and social services directly from providers like Mysti's, without being placed into a specific care management program or CCO.
I'm in a CCO — what do I do?
If you are currently enrolled in a Community Care Organization (CCO), you should contact your CCO directly to inquire about HRSN eligibility. CCOs are responsible for determining your eligibility and coordinating your care within their specific program.
What housing help is covered?
We provide rental assistance for up to six months per household. This includes utility help and home changes for safety and health. We also offer outreach and engagement services to help you navigate the Oregon Health Authority's enrollment process.
Does this pay my rent forever?
No. The rental assistance provided by HRSN is a one-time benefit per household. Once this benefit is exhausted, you will need to transition to other housing resources or programs to continue receiving rental support.
When does intake open?
Intake for the Oregon division is currently open for new referrals. We are actively working with referral partners across the Portland metro area to coordinate outreach and engagement services for those who qualify.
