Information We Collect: We collect personal information such as your name, email address, phone number, and mailing address when you join our interest list or contact us directly. This information is used to maintain your profile and communicate with you regarding our services and intake openings.
How We Use It: We use your information to provide outreach and engagement services, including helping you submit your OHP open card enrollment request to the Oregon Health Authority. We may also use your information to coordinate referrals with hospitals, clinics, shelters, and reentry programs.
Sharing Information: We do not sell your personal information. We may share your information with Oregon Health Authority (OHA) and Acentra Health to verify your eligibility for HRSN benefits. We may also share information with our referral partners to coordinate services for people experiencing homelessness.
